The Vermont Agency of Education (AOE) has opened sign-offs for the SY25–26 End-of-Year Data Collection.
The deadlines are:

| Superintendent | September 15, 2026 |
Both signatures must be completed within two business days after the DocuSign envelope is issued.
Locate the Sign-Off Report
The sign-off report is available on the EFT server in the following location:
AOE → Ed-Fi → 2026 → reports → SU0XX-SU-SignOff
Replace SU0XX with the applicable supervisory union number.
Complete the Sign-Off Process
1. Review the report
The Data Manager and Superintendent must both review the applicable SU0XX-SU-SignOff report.
AOE will not issue the DocuSign envelope until it receives confirmation that both individuals have completed their review.
2. Request the DocuSign envelope
After both reviews are complete, the Data Manager should email AOE.SLDSSupportGroup@vermont.gov using the following request:
The Superintendent and I have reviewed our End-of-Year data and the Sign-Off report for SU0XX and are ready to finalize the SY26 End-of-Year Collection via DocuSign.
Remember to replace SU0XX with the correct supervisory union number.
3. Complete both signatures
AOE will send the DocuSign envelope to the Data Manager first. After the Data Manager signs, the envelope will be routed to the Superintendent.
Teams should coordinate availability in advance to ensure:
- The Data Manager signs by September 11, 2026.
- The Superintendent signs by September 15, 2026.
- Both signatures are completed within two business days of the envelope’s issue date.
Avoid Revisions After DocuSign Is Issued
Review the report carefully before requesting the DocuSign envelope.
If revisions are needed after the envelope has been issued, the request requires formal notification to the Superintendent, the DMAD Director, and the Deputy Director. Changes also require approval from the AOE Chief Strategy and Accountability Officer.
To request a revision, email AOE.SLDSSupportGroup@vermont.gov and include the supervisory union name and number. Do not email personally identifiable student information.
